Statement: My NDIS plan is plan managed or self-managed with current funding available

through my NDIS plan for dates of service required and stated above. The standard NDIS

rates of support respite (24 hours) is charged as outlined in the NDIS price guide. The NDIS

cancelation policy also applies.
